Early registration for women planning on giving birth at Shaare Zedek is highly recommended as it speeds up the entire process of admission. Upon your arrival, all your important medical data will be available to the team at the click of a button.

Please print, clearly fill out and fax this (HEBREW)form , together with photocopies of both parts of your ID card and your partner's (without which the process of early registration cannot be carried out) to:
