wPCDAI (weighted Pediatric Crohn's Disease Activity Index) 1-3

Aim: to serve as an overall multi-item measuring tool of disease activity in pediatric Crohn’s disease

The Pediatric Crohn's Disease Activity Index (PCDAI) has become the standard for measuring disease activity in pediatric Crohn's disease. In a multi-variable model, three items were shown to be redundant (i.e. height velocity, abdominal examination and hematocrit level), thereby improving its feasibility and responsiveness. Moreover, the construct validity and reliability of this weighted PCDAI (wPCDAI) were improved with better differentiation between remission and active disease, and between moderate and severe disease.  Since the improved feasibility did not come at the expense of reduced performance, multiple studies have been utilizing the wPCDAI since its introduction.

Cutoff values of wPCDAI to determine disease activity: 

wPCDAI < 12.5 (remission)
wPCDAI 12.5-39 (mild disease)
wPCDAI 40-57 (moderate disease)
wPCDAI ≥57.5 (severe disease). 

A change of at least 17.5 points is the minimally important difference and defines meaningful response.
