UGI-SES-CD (Upper gastrointestinal simple endoscopic score [SES] for Crohn's disease [SES-CD])1
Aim: To develop a standardized endoscopic inflammatory description of the upper gastrointestinal (UGI) tract in Crohn's disease.
Noting that upper gastrointestinal (UGI) inflammation in Crohn's disease may be associated with a more severe phenotype, the simple endoscopic score (SES) for Crohn's disease (SES-CD) was modified to include the UGI tract (SES-CD-UGI). The score ranges from 0 to 17 and is based on the size of ulcers, ulcerated surface, affected surface, and narrowing. During its validation, the UGI-SES-CD score was associated with the weighted Pediatric Crohn's disease activity index (wPCDAI), Physician's Global Assessment (PGA) of inflammation, ileocolonoscopic SES-CD, fecal calprotectin, and radiological global assessment of damage by magnetic resonance enterography (MRE).
The four UGI segments (esophagus, gastric body, gastric antrum, and duodenum) are scored separately. Each variable should be scored between 0 and 3, as per the table below, for each of the four UGI segments. Each segment should be scored based on the worst area in that segment.
In the event that findings are unrelated to Crohn’s disease, such as Helicobacter pylori gastritis, reflux esophagitis or celiac disease, these should be disregarded and the UGI-SES-CD in these regions should be scored as normal. When in doubt, the findings should be attributed to Crohn's disease.
