The Shaare Zedek Medical Center (registered association) website ("Shaare Zedek" and the "Website"), contains, among other things, the "Donations" page (the "Donations Page") through which you may donate to Shaare Zedek. These terms (the "Terms") govern your use of the Donations Page. Any use or attempt to use the Donations Page, including for the purpose of making donations, constitutes consent on the part of the person making or attempting to use the Donations Page ("User") as stated in these Terms, which form part of the Website’s “Disclaimer - Terms of Use” (the "Terms of Use"), and an undertaking to abide by these Terms and the Terms of Use.
1.A User who is over the age of 18 and/or a legal entity incorporated by law, who has a valid credit card and the card is in the User's name, may donate to Shaare Zedek through the Donations Page, subject to the provisions of these Terms. The Donations Page is secured according to the PCI DSS standard.
2.When making a donation using a credit card, the card will be checked with the applicable credit card company and the User will receive a response regarding the status of the transaction and whether it was accepted or rejected.
3.In the event the donation is not approved by the credit card company, the User will receive appropriate notification. In order to complete the donation, please make telephone contact with Shaare Zedek.

5.For each donation received, a receipt will be sent to the donor to the postal address entered by the User, along with a recognized tax exemption certificate in accordance with Section 46 of the Israel Income Tax Ordinance.
6.A User may request that Shaare Zedek cancel a donation within 14 days following the day the donation was made through the Website. Shaare Zedek will comply with such request in accordance with the relevant credit card company's policy in force at the time. If the credit company charges Shaare Zedek a fee for such cancelation, such fee will be charged to the cardholder.
7.Privacy. (A) There is no obligation to make a donation or provide any information on the Donations Page. However, there is certain information which if the User does not provide, the User will not be able to make a donation; (B) Shaare Zedek does not keep Users' credit information (these are provided to the applicable credit card company, in accordance with their rules and procedures), but uses the information provided by the User to communicate with him, execute the donation, protect Shaare Zedek's rights and perform its activities; (C) Shaare Zedek will not transfer information about Users to third parties except for the purpose of realizing the aforesaid.
8.The User represents and warrants as follows:
8.1The User is over the age of 18 or a legal entity incorporated by law, qualified to perform the actions it seeks to perform on the Donations Page.
8.2The credit card on which the User wishes to make a donation is valid and registered in the User's name.
8.3The User's use of the Donations Page (including for the purpose of making a donation), will not violate the provision of any law or the rights of another person.
8.4The personal information the User provides will be correct, complete and accurate. Providing false, incorrect or inaccurate information may result in damages to Shaare Zedek, and in some cases may constitute a criminal offense.
9.Limited Liability. Shaare Zedek shall not bear any liability or responsibility in connection with any action performed by a User on the Donations Page or for any other party that is not under the Shaare Zedek's sole control, and shall not be liable for any damage and/or loss caused in connection with the aforesaid. The full and exclusive responsibility for the use of the Donations Page, including any error in entering information, shall be borne by the User, and the User will bear any consequence or damage (to the User, to Shaare Zedek and/or to others) that may result thereof. The computer records, as they appear in Shaare Zedek's systems, will constitute evidence that the information appearing in them has been entered by the User.
10.These Terms are part of the Terms of Use and it is clarified that clauses such as "Judicial Authority" and "Changes to the Site and Cessation of Services" will also apply to the Donations Page and the User's activity therein. It is also clarified that the representations and obligations of the User in these Terms are intended to add to and not detract from the Terms of Use. Shaare Zedek reserves the right to change the provisions of these Terms from time to time, with immediate effect.
