Please come 15 minutes before your scheduled appointment to take care of the payment.
Patients who have Clalit Mushlam insurance should state this when scheduling their appointment. 
Patients with private medical insurance are eligible for a full or partial refund, in accordance with the conditions of their policy. 
Patients with supplementary insurance should check their eligibility for a partial refund.
