Medical Records

As a general rule, you should avoid requesting a patient's record while they are still hospitalized, unless the patient or their representative is seeking a second opinion.

The patient must sign a medical confidentiality waiver for their representative.

• For minors: When the request is on behalf of a minor, the natural guardian (mother or father) must sign the medical confidentiality waiver. It is recommended to have both parents sign and to specify the purpose of the request.
  
  

• For mentally ill, unconscious, or other vulnerable individuals: The legal guardian is the only one authorized to sign the confidentiality waiver. The guardian must provide a certified true copy of the guardianship order.

• A fertility file belongs to both partners (typically).
  
  

• To obtain medical documents, you must complete the "Request for Fertility File" form and attach a copy of an ID.
  
  

• If only one partner will be collecting the documents, a confidentiality waiver signed by the other partner must be included.
  
  

• If a third party is collecting the documents, a confidentiality waiver signed by both partners for that third party must be included. The third party must also bring a copy of both partners' IDs.
  
  

• After submitting the request, a representative will contact you to coordinate the pickup time, delivery method, and payment.

• A doctor requesting medical information for the purpose of continuing a patient's treatment should send an inquiry to the department via mail, fax (02-6666911), or email.
  
  

  The request must be on official letterhead with the institution's logo, and include the signature and stamp of the requesting doctor or "Tipat Halav" (Well-Baby Clinic) nurse.
  
  

  The inquiry letter must include the following details:
  
  

  • Patient's last and first name
    
    

  • Identity card number
    
    

  • Required information: A discharge letter from a hospitalization department, the emergency room (please specify visit date), or a clinic (please specify clinic name and letter date)
    
    

  • Purpose of the request
    
    

  There is no fee for a treating doctor's request for medical information.

According to the procedure for releasing information about a deceased person, information can be provided to the deceased's heirs upon presentation of one of the following documents:

• Probate order
  
  

• Will and a probate order
  
  

• Court order
  
  

• A sworn statement from the applicant, legally verified and signed by a lawyer or court registrar, declaring that they are the heir of the deceased and that there are no other heirs, or that all heirs agree to the release of the information, and that the applicant does not object to the release of the information to other heirs.
